Electronic cigarettes are still so new on most markets, many languages haven't yet decided what to call the users.

Should they be called "vapers," as some American smokers dub their colleagues who smoke smokeless cigarettes?

The word makes sense. If smokers are named after smoke, then "vapers" should be named after the nicotine vapor that electronic cigarettes emit instead.

Whatever one calls smokeless smokers, their number is growing rapidly.

A recent study by the British health campaigning charity Action on Smoking and Health (ASH) found that the number of people in that country who reported trying electronic cigarettes more than doubled, from 9 percent in 2010 to 22 percent in 2012.

Almost all the new users were people who already had smoked cigarettes and 40 percent of them hoped switching would free them from the usual hazards of smoking.

Those hazards include an increased risk of dying from cancer, mostly due to inhaling the tar and other toxins released by tobacco when it is burned.

How Much Safer?

But are electronic cigarettes, or e-cigarettes, really safer than smoking?

Amanda Sandford, a research manager for ASH in London, says the only tobacco product the new devices contain is nicotine.

"Nicotine, compared to tobacco smoke, is relatively benign. It is the addictive component of tobacco, of course, and that is what keeps people coming back to smoking, but the harm [in smoking] comes largely from the inhaling of tobacco smoke," Sandford says.

"Nicotine itself, once it is isolated and extracted from tobacco and just used in its pure state, is relatively harmless."

Most users of electronic cigarettes surveyed believe they're less dangerous than regular cigarettes and will help them stop smoking.
Some 80% of individuals who reported using so-called electronic cigarettes in a large survey said the products are safer than traditional cigarettes. Battery-powered e-cigs, also known as electronic delivery nicotine systems, heat and vaporize a solution containing nicotine, usually with flavorings added. Many are designed to resemble traditional tobacco cigarettes. Most e-cigarette users also said they buy the products to help them reduce or quit smoking, according to Richard O'Connor, PhD, of Roswell Park Cancer Institute in Buffalo, N.Y., and colleagues. The survey was conducted in nearly 6,000 current and former smokers in the U.S., Canada, Australia, and Great Britain. About half of all respondents had heard of e-cigarettes, but only 3% reported current use of the products. Health Concerns for Electronic Cigarettes

Proponents of electronic cigarettes often claim that electronic cigarettes deliver the experience of smoking while eliminating the smell and health risks associated with tobacco smoke. The base liquids - which include propylene glycol (PG), vegetable glycerin (VG), and sometimes polyethylene glycol 400 (PEG400) - have been widely used as a food additive, as a base solution for personal care products such as toothpaste, and in medical devices such as asthma inhalers. However, the health effects of inhaling nicotine vapor into lungs are a subject of uncertainty. The health effects of exhaled nicotine and other substances present in the vapor are also uncertain but seem likely to be far less harmful than tobacco smoke.

The fact that e-cigarettes may resemble real tobacco cigarettes has been noted by both supporters and detractors. While e-cigarettes may give nicotine addicts more or less the same amount of nicotine as a conventional cigarette, they do not produce the same toxic smoke that can cause lung disease and cancer when inhaled over time. Since there are no products of combustion to be inhaled, no tobacco toxins are inhaled besides nicotine.

AAPHP Statement re State Regulation of E-cigarettes


The American Association of Public Health Physicians recommends the following State response to proposed legislation to ban or otherwise restrict the sale and use of nicotine vaporizers (commonly referred to as E-cigarettes or electronic cigarettes).

1. Sale to adults should be permitted.
2. Sale to minors should be banned.
3. AAPHP takes no stance on the question of whether E-cigarettes should be banned in no-smoking areas. (see explanation)

An E-cigarette is not a cigarette. It is a metal tube made to look like a cigarette, with a battery, heating element and cartridge containing the substance to be vaporized. The substance is usually a mixture of propylene glycol, glycerin, flavoring, and a specified quantity of nicotine. When the vaper (person using the E-cigarette) inhales, an LED lights up to make the device look more like a cigarette. When he or she exhales, there is a visible cloud of vapor that disappears within a few seconds. 

Neither I (Dr. Joel Nitzkin) nor the organization I represent (the American Association of Public Health Physicians) have received or anticipate receipt of any financial support from any E-cigarette, tobacco-related or pharmaceutical enterprise. 

AAPHP favors a permissive approach to E-cigarettes because the possibility exists to save the lives of four million of the eight million current adult American smokers who will otherwise die of a tobacco-related illness over the next twenty years. 

The only feasible way to achieve this remarkable public health benefit will be to inform smokers of the differences in risk posed by different categories of nicotine-delivery products. Conventional cigarettes account for about 80% of nicotine consumption in the United States, but more than 98% of the illness and death. This harm is not caused by the nicotine, but by toxic products of combustion. A cigarette smoker can reduce his or her risk of future tobacco-related death by 98% or better by switching to a low risk smokeless tobacco product. He or she could cut that risk by 99.9% or better by switching to a nicotine-only delivery product like one of the pharmaceutical products or E-cigarettes. 

Experience suggests that E-cigarettes may be more acceptable to smokers than the currently available pharmaceutical alternatives. A smoker can secure almost all the health benefits of quitting if he or she transitions to an E-cigarette. 

Quitting, of course, is best. About 3% of smokers succeed in quitting each year. Pharmaceutical smoking cessation products, when used as directed, can increase that to about 7%. Thus, the current pharmaceutical products fail 93% of those who try them, even with the best of health education and counseling. Long term use of an alternative nicotine delivery product can achieve almost all of the benefits of quitting for those unable or unwilling to quit. E-cigarettes can and should be marketed as a substitute for conventional cigarettes for smokers unable or unwilling to quit. 

State legislatures and, hopefully the FDA should see them in this light and regulate their marketing to reflect this purpose. Given the current lack of federal regulation, some, but not all, E-cigarette vendors adhere to this guideline. 

Sales to minors should be prohibited. If someone does not become addicted to nicotine as a minor, it is unlikely that he or she will ever become addicted.

E-cigarettes deliver the same nicotine found in the pharmaceutical products, with no more contamination by toxic substances than the pharmaceutical products already approved by FDA. Propylene glycol and glycerin are used as American Association of Public Health Physicians carriers of the nicotine. These cause the visible vapor. These substances are generally recognized as safe. They are
commonly used in theatrical fog machines, asthma inhalers and air fresheners. There is no smoke, and no products of combustion. All this creates a situation in which we can confidently state that the risk to others sharing an indoor environment with one or more vapers (E-cigarette users actively using this product) is almost sure to be much less than 1% the risk posed by environmental tobacco smoke. Pharmaceutical nicotine vaporizers have been in use for years, with no visible vapor, and no apparent concern about use in non-smoking areas. This having been said, we cannot rule out the possibility that some individuals who may be extremely sensitive to indoor air irritants or to miniscule concentrations of nicotine in indoor air might be adversely affected by E-cigarette (or pharmaceutical nicotine vaporizer) vapor.

Another issue is that of modeling. Some worry that sight of E-cigarettes in non-smoking areas will make smoking restrictions harder to enforce, or encourage minors to see smoking as a “normal” and acceptable behavior. It is important to note that, on second glance, E-cigarettes are easy to distinguish from tobacco cigarettes. Those seeing this as a major issue are inclined to ban use of E-cigarettesin non-smoking areas.

We therefore recommend that research be done to address these two issues (possible hazard to a very small number of highly sensitive individuals and modeling). The problem here is that, with end points so difficult to document, such research could cost millions of dollars and take many years to complete. For the reasons noted above, we do not offer a stance in favor of or against banning E-cigarettes in non-smoking areas.

Since many of my patients have reported using electronic cigarettes to successfully stop smoking, I now recommend the devices to anyone who has tried to quit smoking cold turkey and failed.
And I think it is time that other doctors do, too.
Electronic cigarettes combine a mouthpiece, which contains liquid (including nicotine), an atomizer (which heats the liquid and turns it into vapor), a battery and an LED tip that glows like the tip of a lighted cigarette.
While early versions of the electronic cigarette date back to 1963, with a patent awarded to inventor Herbert Gilbert, the modern versions of electronic cigarettes—the basis for big brands in the industry, such as LOGIC and Blu—were introduced at the beginning of this century.
The reason my patients tell me electronic cigarettes work better than the patch or nicotine gum is that they simulate the act of smoking, but not perfectly. They are good enough to substitute for real cigarettes, but they aren’t good enough to become an addiction, in and of themselves. An analogy in the arena of food addiction would be something low calorie that fills you up enough to prevent bingeing on sweets, gives you some distance from that addiction, but then becomes forgettable, because it isn’t really all that compelling.
It is, of course, imperative that the electronic cigarette be a good-enough fake. And, on this count, LOGIC seems to have a slight psychological advantage, given what patients tell me is a very realistic smoking experience—but not too realistic, as noted above. Interestingly enough, the LOGIC brand seems to be the best-selling one in New York City, perhaps because of these factors.
There is certainly controversy about whether electronic cigarettes are harmless. Critics note that they do, of course, contain nicotine (which is the whole idea, after all). And critics have also found other substances in the vapor released by electronic cigarettes—even cancer-causing substances, but in tiny, tiny amounts that proponents of the devices claim would have no negative effect on well-being at all.
What no one seems to argue about is that electronic cigarettes—from LOGIC or Blu or any leading brand—are not nearly as dangerous as smoking real cigarettes. LOGIC claims its device avoids 4,000 toxins that are found in cigarettes.
Given my experiences and those of numerous clinicians I have spoken with, it would seem to be a good time to conduct large scale clinical trials in which patients who smoke are given electronic cigarettes by their doctors, encouraged to use them and then quizzed on their use of real tobacco weeks and months and years later. If the data generated support the product, then it may be wise for medical insurance companies to offer electronic cigarettes to smokers for free. My bet is they would save lots of money—from the costs of treating heart disease and cancer—down the road.

What is E-liquid?
Answer:E-Liquid is a nicotine solution sold separately for use in refillable cartridges referred to as "e-liquid" or "e-juice", and commonly contain some amount of flavoring, with several different flavors available. They consist of nicotine dissolved in propylene glycol (PG) and/or glycerin (glycerol). Both PG and glycerin are common food additives.

How do I refill a cartridge using Beyond E-liquid?
To refill a standard cartridge Insert a paper clip into the end of the cartridge where the small hole is. This will eject a smaller "Carrier cartridge" This cartridge contains a small amount of cotton like material that holds the E-liquid. Simply drip a few drops of liquid into the Carrier Cartridge, insert it back into the cartridge (don't push it in to far). You now have a refilled cartridge. Be careful not to spill or splash the e-liquid on your hands or face. Wash your hands when you are finished. Avoid contact with your eyes or mouth.

How do I refill a Cartomizer using Beyond E-liquid?
To refill a Cartomizer simply remove the white cap (the end that would go in your mouth) a small screw driver will do the trick. Simply drip a few drops of liquid into the Cartomizer, replace the small white cap, You now have a refilled cartomizer. Be careful not to spill or splash the e-liquid on your hands or face. Background
Electronic cigarettes (e-cigarettes) are battery-powered devices that deliver nicotine without any combustion or smoke. These devices have generated much publicity among the smoking-cessation community and support from dedicated users; however, little is known about the efficacy of the device as a smoking-cessation tool.
Purpose
This study aimed to examine the effectiveness of e-cigarettes for smoking cessation using a survey of smokers who had tried e-cigarettes.
Methods
Using as a sampling frame a cohort of all first-time purchasers of a particular brand of e-cigarettes during a 2-week period, a cross-sectional, online survey was conducted in 2010 to describe e-cigarette use patterns and their effectiveness as a smoking-cessation tool. There were 222 respondents, with a survey response rate of 4.5%. The primary outcome variable was the point prevalence of smoking abstinence at 6 months after initial e-cigarette purchase.
Results
The primary finding was that the 6-month point prevalence of smoking abstinence among the e-cigarette users in the sample was 31.0% (95% CI=24.8%, 37.2%). A large percentage of respondents reported a reduction in the number of cigarettes they smoked (66.8%) and almost half reported abstinence from smoking for a period of time (48.8%). Those respondents using e-cigarettes more than 20 times per day had a quit rate of 70.0%. Of respondents who were not smoking at 6 months, 34.3% were not using e-cigarettes or any nicotine-containing products at the time.
Conclusions

Electronic cigarettes, an increasingly popular option among smokers trying to quit, do not appear to pose a threat to the heart, according to results of a clinical study presented on Saturday.
Greek researchers said e-cigarettes - battery-powered metal tubes that transform liquid laced with nicotine into vapour - had no adverse effects on cardiac function in their small trial.
"Electronic cigarettes are not a healthy habit but they are a safer alternative to tobacco cigarettes," Dr Konstantinos Farsalinos from the Onassis Cardiac Surgery Center in Athens told the annual meeting of the European Society of Cardiology.
"Considering the extreme hazards associated with cigarette smoking, currently available data suggest that electronic cigarettes are far less harmful and substituting tobacco with electronic cigarettes may be beneficial to health."
Farsalinos and his team examined the heart function of 20 young smokers before and after smoking one tobacco cigarette against that of 22 e-cigarette users before and after using the device for seven minutes.
While the tobacco smokers suffered significant heart dysfunction, including raised blood pressure and heart rate, those using e-cigarettes had only a slight elevation in pressure.
The Greek clinical study was the first in the world to look at the cardiac effects of e-cigarettes. Another small study, also in Greece, reported earlier this year the devices had little impact on lung function.
Farsalinos acknowledged bigger studies were still needed to examine the possible long-term effects of e-cigarettes, while other doctors attending the medical meeting in Munich were cautious about giving them a clean bill of health just yet.
"Obviously, the e-cigarette has the advantage of not having the thousands of other chemicals, besides nicotine, that a real cigarette has," said Dr Russell Luepker of the University of Minnesota.
"I don't think it's conclusive but there's no doubt if you expose someone to fewer bioactive chemical compounds there is going to be less effect."
E-cigarettes were first made in China in 2003 but are now sold around the world and used by millions of people.

Switching to e-cigarettes, or electronic cigarettes, from traditional tobacco cigarettes may help smokers quit smoking. Furthermore, even substituting a few tobacco cigarettes with e-cigarettes may offer modest improvements in health, says a new study in a working paper from independent university researchers at the Tobacco Harm Reduction project.
E-cigarettes are a device that deliver nicotine to the user by vaporizing a gel composed of water, propylene glycol, flavorings, and nicotine. Because the devices do not expose users to the products of combustion associated with tobacco cigarettes, e-cigarettes are touted as a safer alternative to smoking. The risk of death associated with e-cigarettes is similar to the risk associated with other smokeless tobacco products, which is approximately 1 percent of the risk of smoking.
The e-cigarette industry is infamous for making the claim that e-cigarettes will help smokers quit smoking. However, such controversial claims are not yet backed up by legitimate research.
For example, as reported by Medical News Today, a press release for e-cigarettes on The Electronic Cigarette Hub proclaims:
“E-cigarettes have already helped thousands of people, including actress Katherine Heigl. Users say it looks and feels like smoking a real cigarette. The E-cigarette even has the glowing red tip; but unlike a tobacco cigarette, it produces an odorless water vapor – there is no smoke; you don’t light it.”
In the present study, researchers investigated the use of e-cigarettes for smoking cessation as well as changes in health including the symptoms caused by smoking. The participants in the study included 303 individuals who were enrolled by e-mail and links on various blogs and forums between May 2009 and June 2009.
Of the total number of participants, 91 percent had made attempts to quit smoking before switching to e-cigarettes. Seven-nine percent were using e-cigarettes instead of tobacco cigarettes. Seventeen percent were using e-cigarettes as a partial replacement for tobacco cigarettes. Four percent were using e-cigarettes in addition to tobacco cigarettes.
Furthermore, 91 percent of the participants report an improvement to their general health as well as an improvement in smoking-related symptoms such as cough, sense of taste, and sense of smell since switching to e-cigarettes. Individuals who had completely switched to e-cigarettes from tobacco cigarettes were most likely to experience health improvements, but substituting even a few e-cigarettes for tobacco cigarettes offered some modest health benefits.
However, the claims that e-cigarettes are less dangerous alternative to tobacco cigarettes and that e-cigarettes can help smokers quit smoking are still controversial and inconclusive. Further research needs to be conducted to evaluate the claims reported in the present study.

The cartridge, a small plastic container with openings on each end, serves as both a liquid reservoir and mouthpiece. It allows the passage of liquid to the atomizer and vapor from the atomizer back to the user's mouth, without leaking liquid into the mouth.
Most models adopt a plastic sponge to keep the liquid in place, but it is also common to use a refillable tank to hold the liquid, with a separate tunnel connecting to the atomizer. When the liquid is depleted, users can refill it or replace with another pre-filled cartridge. Some users forgo the use of liquid reservoirs and drip liquid directly onto the atomizer in a method known as "dripping".

The atomizer contains a small heating coil that vaporizes the liquid, and generally consists of a simple filament and wicking metal mesh or silica wick to draw the liquid in. It is positioned in the center of the three components that make up the entire electronic cigarette cylinder, as the cartridge attaches to one end, and the power unit to the other. The atomizer's filament will lose efficiency over time due to a buildup of sediment, or "burns out" entirely, requiring replacement. In some models, the cartridge and atomizer component are integrated, known as a cartomizer.

Liquid for producing vapor in electronic cigarettes, known as e-juice or e-liquid, is a solution of propylene glycol (PG) and/or vegetable glycerin (VG) and/or polyethylene glycol 400 (PEG400) mixed with concentrated flavors, and optionally, a variable percent of a liquid nicotine concentrate.
They are often sold in a bottle or as pre-filled disposable cartridges. Battery:
